Do I need a physician referral for physical therapy?

You do not require a referral for physical therapy in Illinois! Please come in for an evaluation and our skilled clinicians can determine if physical therapy can help you!

How do I prepare for my first appointment?
​

Once you are scheduled for your evaluation, whether you scheduled online or over the phone, you will receive an email including paperwork to complete prior to your appointment. We recommend completing this ahead of time to maximize your time with the therapist. Please bring an ID and insurance card to your appointment.

Can I bring my child to my appointment?
​

Yes! We completely understand how hard it can be to find a sitter. We can use this as an opportunity to go over child care activities that induce pain.

What do I wear to my appointment?

​

We recommend wearing something loose-fitting and comfortable to your appointment since we will be moving and stretching.

Do you provide parking?

Free street parking is available on California Ave. and the nearby streets!

Do you take insurance?
 

We are in-network with BlueCross BlueShield PPO, BlueCross Blue Choice PPO, Anthem BlueCross BlueShield PPO, and Medicare plans. If you have another insurance, we can submit it on your behalf for out-of-network reimbursement. Please call us for more information. Self pay is also available.
